After you have completed your pre-enrollment, please complete this UHC Family Intake Form. This information will be discussed as we walk through the face to face interview process. Your responses help us serve and care for your students in a variety of ways. Because it is so vital in helping us get to know your family, you will not be able to proceed with your on site enrollment appointment until it is completed.

Please note that this intake form will take some time to complete depending on how many students you are enrolling. You can save your progress and come back to this at a later time if needed within 30 days of starting.

You will be required to sign up for a time to have your child's yearbook photo taken at your enrollment appointment. These photos are used both for the yearbook as well as photo IDs for 3-12th graders and bag tags for K3-K5 students. Please be prepared with your calendar to book this appointment and pay at your enrollment appointment. This fee is separate from your UHC tuition and will need to be paid via debit/credit card.
