Parent Assistant Info

Each family at UHC is required to serve as a parent assistant for a certain number of service hours each academic year. New and current members should reference the Parent Service Agreement for more info on required hours and what is expected.

Background Check Form:

All Parent Assistants serving on campus must complete a Background Check form before serving on-site.

Nursery Forms (complete if applicable):  

The UHC nursery teachers want your little ones to have the best experience possible while in their care, so to help them know your child, please be sure to complete a Child Info Form. This will allow the teachers to view your child’s information before their first day.
