UHC Parking Permit

Please complete this form to register your vehicle on campus and to receive or renew a UHC parking permit. Please fill out a NEW FORM for EACH parking permit if you wish to renew or purchase additional parking permits.

New UHC Parking Permits: Cost $5 per new parking permit. All parking permits will be paid for and distributed at enrollment appointments or other designated times communicated by UHC staff.

Parking Permit Renewals- for returning members who already have a parking permit, renewal of permits will cost $1 per permit annually. An updated parking permit sticker will be paid for and distributed at enrollment appointments or other designated times communicated by UHC staff.
