UHC Parking Permit

Please complete this form to register your vehicle on campus and to receive or renew a UHC parking permit. Please fill out a NEW FORM for EACH parking permit if you wish to renew or purchase additional parking permits.

New UHC Parking Permits: Cost $5 per new parking permit. All parking permits will be paid for and distributed at enrollment appointments or other designated times communicated by UHC staff.

Parking Permit Renewals- for returning members who already have a parking permit, renewal of permits will cost $1 per permit annually. An updated parking permit sticker will be paid for and distributed at enrollment appointments or other designated times communicated by UHC staff. You must bring your physical tag to enrollment in order to renew it. If you are unable to locate your tag to bring to enrollment, you will be required to purchase a new tag.
