Please complete this form to register your vehicle on campus and to receive or renew a UHC parking permit. AFTER COMPLETING THE FORM, PLEASE SEE YOUR CAMPUS MANAGER TO PAY AND PICKUP YOUR PARKING PASS! Please fill out a NEW FORM for EACH parking permit if you wish to renew or purchase additional parking permits.

New UHC Parking Permits: Cost $5 per new parking permit. All parking permits will be paid for and distributed at enrollment appointments or other designated times communicated by UHC staff.

Parking Permit Renewals- for returning members who already have a parking permit, renewal of permits will cost $1 per permit annually. An updated parking permit sticker will be paid for and distributed at enrollment appointments or other designated times communicated by UHC staff. You must bring your physical tag to enrollment in order to renew it. If you are unable to locate your tag to bring to enrollment, you will be required to purchase a new tag.

If I desire to renew my existing parking permit, I understand I must bring that tag to enrollment day to renew it. I understand that if I am unable to bring my tag to renew, I will be required to purchase a new tag for $5. This applies to any parent or student parking permit I wish to renew.(Required)
